Bankruptcy Checklist:

In the event that you decide to file bankruptcy after meeting with us, you’ll need to provide our office with the following information:

  • 6 month income history
  • 2 most recent state and federal tax returns

Once you’ve met with us, you’ll also need to return several items that we will send by mail:

  • Retainer Agreement
  • 2 most recent state and federal tax returns
  • Completed debtor information packet
  • signed Section 342(b) notice
  • signed Section 527(a) notice
  • signed Section 527(b) disclosure

Prior to filing, you’ll also need to complete a certified credit counseling course.  You can take that course through Rural Services of Indiana, Inc. here or from any other entity that offers the certified course.

Upon filing, you must also take a Personal Financial Management Workshop course in order to successfully receive a discharge. That course also is offered by Rural Services of Indiana, Inc. here but may be obtained from any other entity that offers the certified course.
